New Dental Patient Information – New City, NY
Thank You for Choosing Our Dental Office
You’re not just another patient at Cosmetic and General Dentistry of New City; you’re a valued member of our dental family, and your comfort and convenience are of the utmost importance to us. Our team is dedicated to making each visit a success, and that starts with the preparations beforehand. Please review the information below and feel free to call us if there’s anything else that you want to clarify before your first checkup or consultation.
Your First Visit
When you arrive for your first visit, we’ll ask you to complete any remaining paperwork (if you haven’t already downloaded the necessary forms). Then we’ll bring you to our treatment area, where we’ll ask questions about your oral health while taking X-rays, examining your teeth, and performing any necessary cleaning. Then we can discuss existing issues and make recommendations for follow-up appointments. We’ll make sure that you’re fully aware of your situation and treatment options before recommending any specific services.
New Patient Forms
Our new patient forms are here for you to download and fill out at home. By completing them in your spare time, you can take a lot of the stress out of your first visit. Instead of getting to our dental office several minutes early to fill out the paperwork, you can simply hand in the forms as soon as you walk in. This way, we can get the information we need right away, and you’ll be able to see your dentist much sooner.
